The Institute for Family Health is seeking a Patient Navigator to work as part of our interdisciplinary, patient-centered HIV care and prevention team in the Bronx and Manhattan. The HIV Patient Navigator will work closely with patients living with HIV that are newly diagnosed, new to treatment or facing barriers to adherence and retention in care. Patient Navigators support the mission of the Institute, to provide care to underserved communities, by outreaching, engaging, intaking, assessing for barriers and identifying solutions, completing care plans, accompanying patients to medical and social service appointments, providing health education, case management and coordinating all aspects of care. Patient Navigators support the mission of the HIV Dept., to provide services using a social justice lens, acknowledging the injustices of health disparities in HIV and LGBTQ communities, which are often inundated with discrimination, violence, racism, systemic oppression, and stigma.
